Data from 75 patients with back pain were reviewed, including 37 who had myelography. 29 of the myelograms were abnormal. In these patients, denervation was most likely present in both limb and paraspinal muscles, and abnormal H reflex or F response studies common. In the remaining patients, denervation was most frequently present in paraspinal muscles only, these findings correlated with abnormal lumbosacral x-rays, and H reflex and F response studies tended to be unrevealing. These differences can be explained by a tendency for more proximal root involvement with disc disease in contrast to more distal entrapment of the dorsal rami with bony changes. The data also indicate that readily available anatomical and physiological information can be used to meaningfully classify patients with back pain.
