Techniques for the amputation of the upper extremities - When any amputation of the upper extremities is performed, consideration must be given to the maintenance of all parts of the extremity which may be able to assume a further function at a later stage. This is particularly important in emergency treatment after injury. Initially, it is often not possible to ascertain whether organ structures within the area of injury can be maintained or whether they will become necrotic tissue. The wound closure has to be performed in such a way that secondary healing is avoided, because this will result in tissue damage, thus disturbing valuable functions and making prosthetic supply difficult. The final modelling of the amputation stump can be better and more efficiently achieved during a second planned operation. Traumatic amputations can often produce good final results, but this cannot be achieved in most cases of thermoelectric injuries. If possible, the final provision should aim to maintain the given stump length. Today, less valuable limb sections, as they used to be schematically termed, no longer exist thanks to modern operative and prosthetic treatment possibilities. Even ultrashort and ultralong stumps can be catered for prosthetically. These allow the injured person to enjoy much better function than shorter extremities would allow.
