Thirty-three patients with stable heart failure due to ischemic or idiopathic dilated cardiomyopathy without signs of myocardial ischemia or inflammation underwent a symptom-limited bicycle exercise test and were compared with 10 control subjects. The exercise-induced increase in cardiac troponin I was correlated with the circulating inflammatory cytokines tumor necrosis factor-alpha, interleukin-1beta, and interleukin-6, and was related to a worse clinical course of heart failure during a follow-up of 18 +/- 3 months.
